.258 Linux Crashes every few hours

Hey.
my system is working on a lenovo T420 with an I5 4gb Ram.
i am running ubuntu 14.04 headless server. this has been running for a long time with excellent performance and i am very happy with it.

the past few weeks the system crashes every few hours and i get this error:

now this hasnt happened before so i have no idea if this is related to a problem with HS3 or a system issue.

i replaced the hard disk, installed a new ubuntu 14.04 (also tried it with 15.10) and installed the mono-complete.

i hope someone can help me here.
currently i am running a cron job to test if hs3 is running and if not , restart it.

i have the entire logs being dumped to a log file so if someone needs a copy, let me know
